Eligibility & Referrals for System of Care (SOC) Services

Children and youth who are at risk or experiencing emotional disturbance are eligible to receive System of Care services. This may include children and youth with mental health issues, behavioral challenges, or other emotional struggles that impact their daily functioning and well-being. Eligibility for services may vary depending on the specific program or organization providing System of Care services, but generally, children and youth who meet certain criteria related to their emotional and behavioral needs may qualify for support and intervention through a System of Care program.

Who Is Eligible to Receive SOC Services?

• Children from 5 to 21 years old experiencing severe emotional disturbances

• Any child receiving or in need of multiple services is eligible for assistance from SOC

• Any child at risk for out-of-home placement

• Any child experiencing diminished function in their physical, cognitive, emotional, behavioral, and/or social functioning

• Families experiencing agency confusion regarding leadership in complex cases

• Children, families, or providers who are experiencing communication difficulties

• Treatment that does not appear consistent or coordinated

• Frequent inpatient hospitalizations

• Children experiencing service plans that have not worked

Anyone connected with the child or adolescent, including:

• Child/Youth

• Family Member(s)

• School Official(s) & Faculty

• Agencies

• Providers

• County Authorities

Please Note: Referrals are encouraged to be completed with the SOC Coordinator, but may also be completed by an outside organization or family member.

All original paperwork must be submitted prior to a meeting being scheduled.

(NOTE: Release must be signed by a parent/guardian and/or the child if they are 14 years of age or older.)
